WISD contributes $2.20 towards $20,000 standard/basic AD&D life insurance for each employee.
You may elect to increase this contribution and to also include your family.

For medical information, go to:  www.trs.state.tx.us, select TRS ActiveCare, select medical benefits & pharmacy benefits, select medical administered by BC/BS, select search for a doctor or hospital, select a product type: TRS ActiveCare, customize search.  At this point just follow the instructions.

Medical deductible runs Sept - August

Eligible dependents include:  a spouse; unmarried natural, adopted, foster, or stepchild younger than 25 years of age with whom they have a regular parent-child relationship--there is no requirement to be a full-time student; unmarried natural, adopted, foster, or stepchild who are mentally retarded or physically incapacitated to such an extent as to be dependent on care and support regardless of age (as determined by TRS)

All ID cards will come in the name of the enrollee with an individual ID number.  It will no longer be your SS#.   To order more cards, call customer service at 1-866-355-5999

Routine Preventative eye care is covered under this plan for a co-pay if visiting an Ophthalmologist.  Medical necessity eye appointments will also be a co-pay if you go to an Ophthalmologist

To look up your prescription to see how much you will have to pay, go to:  www.trs.state.tx.us, select TRS ActiveCare/Funding, select Medical & Pharmacy Benefits, select Pharmacy Benefits.  At this point select your ActiveCare plan and follow the instructions.

There is a $50 deductible per year per person for prescriptions.

Employees can only make "life event" changes through the year if you are in the cafeteria plan.  Any changes must be made within 30 days of the event

To check your benefits:  www.ameritas.com   select Ameritas Group, select plan member, select dental benefit summary.  You can enter your information

Unmarried Children will be covered until age 25 regardless of school

If you have another dental plan and have made your deductible for the year, Ameritas will honor the deductible for this calendar year on Major procedures only.  You will need to show proof of making the deductible

Dentures are covered under major on BOTH plans

X-rays are covered at 100%.  It is a separate charge and reimbursement than the exam charge
