Medical

Eligible
dependents include: a spouse; unmarried natural, adopted,
foster, or stepchild younger than 25 years of age with whom they have
a regular parent-child relationship--there is no requirement to be a
full-time student; unmarried natural, adopted, foster, or stepchild
who are mentally retarded or physically incapacitated to such an
extent as to be dependent on care and support regardless of age (as
determined by TRS)

ID cards will come in the name of the enrollee with an individual ID
number. It will no longer be your SS#. To order more
cards, call customer service at 1-866-355-5999

Routine
Preventative eye care is covered under this plan for a co-pay if
visiting an Ophthalmologist. Medical necessity eye appointments
will also be a co-pay if you go to an Ophthalmologist

ActiveCare/Funding, select Medical & Pharmacy Benefits,
select Pharmacy Benefits. At this point select your
ActiveCare plan and follow the instructions. |
